Telecom Requests are processed by this office. The following forms should be completed and sent to NSSB:

Telephone equipment and service, FAX machines, telephone answering machines: DOE TR Form Rev. 8/10 Approval or disapproval is determined by NSSB.

Mobile radios/walkie-talkies: Department of Accounting and General Services/Information and Communications Services Division Telecom Request Form. Approval or disapproval is determined by ICSD
